At PRO HEALTH CARE SERVICES we are committed to improving the quality of life of those who we serve, we understand the mixed emotions, stress and anxiety that can accompany the shift to managing ones own health needs. In our efforts to ease those worries and meet our service goal we have created a system that considers each patient as an individual and allows us to create a customized care plan to satisfy each individual's needs. There is no age limit in order to receive our services, we provide care from pediatric cases to seniors.
From the moment of discharge, we maintain clear communication and coordination with physicians, referral sources, patients and their families to insure that we are constantly providing the right kind of care for the patient. We strive to continually educate patients and their families about their particular health conditions, teaching them how to manage those conditions and when possible helping them become independent again.
