Locate and click on the link titled "Download Now" in the Document download section located immediately below. Upon doing so if you are unable to view the document (which is in PDF form) please get the latest version of Adobe Acrobat Reader by clicking here. Once you are able to view the application, please print and fill it out with the appropriate information remembering to sign where ever a signature is required.
Taking the Tests
In order to be considered for hiring you must take a series of short tests that will be used to assess your current level of knowledge concerning common skills that are commonly needed as a home health aide. To take these tests please visit the PHC eLearning Site at http:// edu.inhomewecare.com. Once there you will need to click on "I do not have an account" and then register with the system. After registering you will need to log back into the system.
You will then click on the picture of the file cabinet that says "course catalog" below it. Then click on the link called "HHA Hiring" course located to the right of a picture of a small stack of books (not one of the two lessons below it).
You should now be looking at info box on the page about that course and in the bottom right of that box you will see a blue curved arrow. Click on it and you will see box to the right titled "Selected Lessons". Click "Continue" and then on the next page click "Free Registration" .
Start with the Homemaking lesson first by clicking on the link. You will see a big green arrow at the top of the page. Click on it and then follow the prompts to take the test.
Assuming you pass the test you will then need to click on the link to the left side of the page that says "change lesson" once you do that you can then click on the other lesson and start the same way. The only change here being that there are 12 sections to this lesson so after you finish each test you will need to click on the little blue arrow at the top left to proceed to the next section. You also have the option of clicking on the different sections in the directory on the right side of the screen.
Once all sections are completed with a passing grade they system will issue a certificate of completion which we can print out at the office.
*Please note: you need to get a score of 70% or better on each section. You can take each test twice so if you don't pass the first time review the test and then try again.
Whats Next?
Once you have completed both the application packet and the online tests please call the office at 614-856-9111 and ask for extention 12. You can then set up a time to come into the office for your interview (if this hasn't happened already). If placement with one or more cases has been determined the hiring process will need to be completed at this time.
Please be sure to bring the following items with you:
- driver's license or valid state ID
- proof of car insurance (if driving)
- proof of any current certifications (ie: First Aid, CPR, ect)
You can also biring with you results from a recent TB test, physical, and BCI background check if you have had any of them within the last 6 months. If you havent then the TB and BCI background check can be done at the office leaving you only responsible to take care of the physical outside of the office.
