Locate and click on the link titled "Download Now" in the Document download section located immediately below. Upon doing so if you are unable to view the document (which is in PDF form) please get the latest version of Adobe Acrobat Reader by clicking here. Once you are able to view the application, please print and fill it out with the appropriate information remembering to sign where ever a signature is required.

Whats Next?

Once you have completed the application packet please call the office at 614-856-9111 and ask for extention 12. You can then set up a time to come into the office for your interview (if this hasn't happened already). If placement with one or more cases has been determined the hiring process will need to be completed at this time.

Please be sure to bring the following items with you:

• driver's license or valid state ID
• proof of car insurance (if driving)
• proof of any current certifications (ie: First Aid, CPR, ect)
• proof of nursing licence

You can also biring with you results from a recent TB test, physical, and BCI background check if you have had any of them within the last 6 months. If you havent then the TB and BCI background check can be done at the office leaving you only responsible to take care of the physical outside of the office.
